NEW!! Guidelines for Community Based Exercise Programs for People with Stroke
There are about 300,000 Canadians living with the effects of stroke. Of those, 25% recover with a minor impairment and 40% are left with a moderate to severe disability (Heart and Stroke Foundation of Ontario 2010). These survivors often struggle to access conventional community fitness programs because of their ongoing physical or cognitive deficits. Yet there is strong evidence that they could benefit from regular exercise programming (Dean et al 2000, Eng et al 2003, 2006, Marigold et al 2005, Pang et al 2005, Stuart et al 2009).
These Guidelines are intended to help exercise providers deliver safe and effective exercise programs to stroke survivors in a variety of community and other settings. These guidelines are congruent with current best practice recommendations and research findings at the time of printing and will be reviewed every three years to reflect evolving practice changes. Recent pilot projects suggest that collaboration between exercise program providers and registered rehabilitation professionals can enhance the safety and design of the exercise program and build fitness instructor skills and competencies (Howe et al 2010, French et al 2008).
Supporting this document is the Guide for Choosing a Community Exercise Program for People with Stroke. This Guide will assist people with stroke and their families in selecting a safe and effective exercise program in the community. Included in the brochure is a checklist to aid in assessing exercise programs as well as other helpful tips. To view the Guidelines and Brochure:

Community Reintegration Questionnaires
Commuity Reintegration Questionnaires have been developed to facilitate the community reintegration of stroke survivors and their family members following hospital discharge. There are three questionnaires:
- The Screening Questionnaire is intended to be used during the initial visit to primary care post-discharge from acute or rehab. The questionnaire was designed to be administered by a care team member (eg, physician, nurse practitioner, RN, case manager). Any questions answered with a "NO" highlight areas of potential need that should be addressed. This questionnaire is designed to target areas of immediate, practical need.

- The Stroke Survivor Questionnaire is intended to be completed by the stroke survivor during a subsequent visit and is designed to identify additional supports that would benefit the stroke survivor and family at a later stage in their recovery journey (eg, three months, six months). This questionnaire may be repeated at various intervals to identify changing needs.

- The Caregiver Questionnaire essentially mirrors the Stroke Survivor Questionnaire with the addition of a few caregiver-targeted questions. The purpose of asking the caregiver to complete a similar questionnaire is to identify where there may be discrepancies between the stroke survivor's perception and the caregiver's perception. This provides an opportunity for diaglogue with the healthcare provider to ensure that all needs are captured. As with the Stroke Survivor Questionnaire, this is to be completed subsequent to the initial primary care visit and may be repeated at intervals as care and support needs evolve.

Interactive Community Resource Database
We have created an interactive database to help clinicians, stroke survivors, care partners and the public to search for the services across Southeastern Ontario that might be useful in post-stroke recovery. We have listed outpatient and community-based stroke rehabilitation services and stroke-related community support services. You can search this database by geographic area and by type of service using an interactive map.
