Health Promotion and Prevention

Secondary Prevention Best Practice Workshop Tool Kit:  Preventing Strokes One at a Time.  To view click here.  This tool kit is designed to support the implementation of the Canadian Best Practice Recommendations for Stroke Care for prevention.  The tool kit provides foundational knowledge required to care for patients who have recently had a stroke or TIA or have been identified as being at high risk.  The tool kit includes an overview and administrative tools for educators, a series of PowerPoint presentations with speakers' notes, and a number of handouts.

A written resource targeted for nurses that reviews pertinent information related to the recognition, prevention, management and recovery from stroke. For more information or to obtain copies, call 1-888-HSF-INFO (1-88-473-4636). Available in English and French.

Dysphagia Management Resources.  A collection of resources related to best practices, screening, managaement addressing both research and education.  Click here.

National Institute of Health Stroke Scale (NIHSS).  A three-hour workshop developed to train front line staff to perform the National Institutes of health Stroke Scale, to provide practice with simulated patients and to use simulations on the training DVD to assess interrater reliability.  Click here.

Canadian Neurological Scale (CNS):  The CNS is a validated and recommended tool for assessing and monitoring the neurological status of stroke clients.  The tools can help you offer a falf day CNS workshop in your organization.Click here to access tools.  Please note that Southeastern Ontario uses the assessment card found here.

A Guide to the Implementation of Stroke Unit Care:  This resources supports the Canadian Best Practice Recommendations for Stroke Care in the acute care setting and inpatient rehabilitation.  It provides guidance for the establishment of new stroke units and the enhancement of exising stroke untis to provide optimal services and processess of care for stroke patients.  Click here.

Stroke Rehabilitation Resource Guide:  The intent of the Stroke Rehabilitation Resource Guide is to enable stroke care providers seeking information related to the rehabilitation of the stroke survivor to access that information, in a user-friendly format, on the Heart and Stroke Foundation of Ontario's Professional Education website. Community and Long Term Care

Tips and Tools for Everyday Living:  A Guide for Stroke Caregivers (2010)
A resource designed to provide the practical knowledge and skills needed by the people who provide care, day in and day out, to the many thousands of stroke survivors living in Ontario. Many of these caregivers are visiting people in their homes, others are providing care in long-term care facilities and still others are working in day programs and other community settings. "The Brain, The Body and You" Workshop Series
"The Brain, The Body and You" pilot project report and full education program is now available on the Heart and Stroke Profed website. Click here to access.

This 5-session interprofessional, continuing education workshop series is designed for regulated and unregulated frontline care providers caring for stroke survivors and other complex client populations in the community, long-term care and other care settings. This continuing education program can be easily individualized by selecting the particular module(s) needed to meet the identified learning needs. It may be offered within existing academic settings or in other educational settings such as the workplace. The 5 modules are each 4 hours in length. The module topics include:
 

  1. Stroke Care - Prevention to Life After a Stroke
  2. Continence Care
  3. Mobility
  4. Nutrition, Swallowing, Feeding and Hydration
  5. Communication and Behaviour

This program is based on the "Tips and Tools for Everyday Living: A Guide for Stroke Caregivers" (HSFO, 2002) and has been made available through a collaborative two-year pilot project involving St. Lawrence College, School of Health Sciences, Kingston, Ontario and the Stroke Network of Southeastern Ontario with funding from the Ministry of Health and Long-Term Care, Ontario Stroke System. To find out more about the pilot project, please click here.
Here in Southeastern Ontario, "The Brain, The Body and You" program will continue to be offered regularly through St. Lawrence College, Kingston with funding support from the Stroke Network of Southeastern Ontario. It is expected that the series will be offered through the College once in the Spring and again in the Fall. Registration announcements will be forthcoming when these program dates are set. Additional requests can be made through St. Lawrence College to:

Education Video Series: Tips and Tools for Everyday Living: A Guide for Stroke Caregivers: Putting It Into Practice
Six education videos have been developed by stroke care champions in Southeastern Ontario. These videos are designed to complement popular sections of the Tips and Tools for Everyday Living: A Guide for Stroke Caregivers (2010) and education programs currently underway across Ontario and Canada. Five of these videos are designed for front line staff and one has been developed for family and community education. Also attached is an advertising poster for the video series.

These educational posters are available 'on loan' at no cost in Southeastern Ontario for display at your location in Long Term Care.  They are available in both wall mount and table top display formats (table top panels can be provided).

The posters have been developed for the education of frontline care providers and are formatted to provide critical learning points in a highly visual way.

Blood Pressure Tool Kit 
This toolkit is designed to assist Personal Support Workers (PSW) and other health care team members to acquire the practical knowledge and skills to support  blood pressure management and to build the capacity to work collaboratively with the health care team and resident/client to optimize blood pressure management. This program is meant to be individualized depending on the needs of the learner and in accordance with the employer/client’s guidelines.  The Learning Plan is divided into two modules each of which can be used independently of the other (Module #1:  The Basics of Blood Pressure & Supporting the Patient and Module #2:  How to Take Blood Pressure).  The toolkit may be used by educators or as self-learning modules.

  The Toolkit Includes:
  1. Learning Plan
  2. Module 1 – The Basics of Blood Pressure and Supporting the Patient provides a theoretical overview of blood pressure including hyper- and hypotension, risks for hypertension, impacts of hypertension and role of the care provider.
  3. Module 1 Case Studies – Three case studies have been developed to support learning in Module 1.  Each case study has a participant and a facilitator version.  The case study questions are designed to demonstrate achievement of the evaluation criteria.

Stroke Nursing Training Modules (Canadian Stroke Network)
The focus of these workshop modules is to compliment the Stroke Best Practice Recommendations and to facilitate nurses’ acquisition of knowledge related to evidence informed practice.  There are four interactive modules addressing a component of the stroke care continuum: Prevention of Stroke, Hyperacute Stroke Management, Acute Inpatient Stroke Care, Stroke Rehabilitation and Community Re-integration.
