Integrating Chronic Disease Prevention with Primary Care in South East Community Health Centres: Building Capacity in Primary Health Care for Reducing Risk of Stroke in HIgh Risk Populations

This project was funded by the Ontario Ministry of Health Promotion, through Ontario Stroke System funding.

The purpose of this project was to increase the uptake of health promotion concepts and the use of that knowledge by the interprofessional primary health care teams in the four Community Health Centres in the South East LHIN.  Learnings from this project will be shared with Family Health Teams and Community Health Centres.

Environmental Scan of LTC Homes in Southeastern Ontario Report, September 2009

In order to better understand how the Stroke Network of Southeastern Ontario (SNSEO) could best support the 39 LTC Homes in our region to implement and sustain best practice stroke care, an environmental scan was conducted. The goals of the scan were to:

  • Inititate or renew contact with the Homes (through the Regional Stroke Community & LTC Coordinator);
     
  • Introduce or re-introduce the available educational supports offered by the SNSEO and obtain feedback on their value;
     
  • Enhance our understanding of the challenges currently faced by LTC Homes and how the SNSEO might help to address these challenges to achieve mutually beneficial outcomes; and
     
  • Inform SNSEO Workplans

Development of the Tips and Tools for Everyday Living: A Guide for Stroke Caregivers - Program for Academic Settings

The objective of this project was to develop a stroke continuing education program for frontline community and LTC health care providers that can be used across existing academic settings and programs throughout Ontario. the program is based on the education resource entitled "Tips and Tools for Everyday Living: A Guide for Stroke Caregivers". The project developed and implemented a series of five focused, 4-hour educational workshops that were piloted at St. Lawrence College, Kingston and Brockville Campuses, Loyalist College in Belleville and a Long Term Care Home.

Rehabilitation Pilot Project

  From 2002 to 2004, the Stroke Rehabilitation Pilot Project of SEO investigated ways to improve the rehabilitation system for stroke survivors, their families and their health-care providers. One part of this project investigated enhancing the timeliness and intensity of community-based rehabilitation services on discharge from inpatient rehabilitation. The results indicated significantly greater change in function in those stroke clients that received the enhanced therapy.
 

Final Report on the Southeastern Ontario Telestroke Pilot Project

The goals of this project were to use telemedicine to support hyper-acute stroke care, to provide consultation services in prevention and rehabilitation and to support health professionals through education.

 


Final Report on the Implementation of a Dysphagia Management Program for Acute Stroke Survivors: The Quinte Health Care Experience

The goal of this project was to ensure the effective and efficient management of dysphagia in those who have experienced acute stroke throughout Quinte Health Care sites.
