Ontario Stroke Report Card FY2009-10, South East LHIN

The LHIN Stroke Report Card provides a high level snapshot of stroke prevention and care.  Page one of the report card includes the indicators/performance measures using a colour coded system to highlight LHIN performance as follows:

Green: Indicates exemplary performance, benchmark achieved or within 5% absolute/relative difference from the benchmark;
Yellow: Indicates acceptable, at or above the 50th percentile and > 5% absolute/relative difference from the benchmark;
Red: Indicates poor performance, with outcomes below the 50th percentile; White: Indicates the benchmark methodology is still in development.

Page two of the Stroke Report Card contains the provincial results. The report is accompanied by an interpretation of the data noting areas of progress/success, opportunities for improvement and planned actions.

Stroke Evaluation Report 2011

This report summarizes provincial and local trends in stroke care indicators from various data sources, including emergency care, acute care, rehabilitation and community stroke care.

Ontario Stroke Audit 2008-09, published March 2011

This report summarizes results of an audit of stroke care in fiscal 2008-09 at all Ontario acute care hospitals. An audit of fiscal 2010-11 charts is now underway.

Consensus Panel on the Stroke Rehabilitation System "Time is Function"
A Report from the Consensus Panel on the Stroke Rehabilitation System to the Ministry of Health and Long Term Care

The purpose of this panel was to develop a framework and key stroke rehabilitation standards for provincial policy development and regional planning as well as evaluation and performance monitoring of stroke rehabilitation services. The region of Southeastern Ontario will be convening 4 local Forums in 2007/08 to discuss this report and the proposed standards and implications for stroke rehabilitation in our region.
