Putting it Into Practice: Lift off Providence Care Outpatient Program is live!

The Stroke Network of Southeastern Ontario wants to congratulate the Providence Care Outpatient Stroke Rehabilitation Program team for their successful program launch! The multidisciplinary team shown above, pictured left to right, includes Olivia Manning (Project Coordinator/Physiotherapist (PT), Zanna Farmer, Occupational Therapist (OT), Jen Tam, Physiotherapist (PT), Nick Van Lier Social Worker (MSW/RSW), Sheryl Gibson, Program Supervisor, and Teresa Whalen, Speech Language Pathologist (SLP) in addition not present for the photo are therapy assistant(s) Allie Palmer, Deanna Mallette, Megan Dijkema (OTA/PTA).  The program will soon include a Communication Disorders Assistant (CDA) another important allied health professional in stroke care.

The Outpatient Stroke Rehabilitation program offers intensive physiotherapy, occupational therapy, speech therapy and social work to support patients who have had a stroke.  Currently, patients are referred from hospital setting (inpatient acute and/or inpatient rehabilitation).  The goal of the program is to help persons with stroke return to what matters most to them after a stroke. Services are tailored to the individual’s needs and may include one or more of the professions listed above.  In addition to tailoring therapy towards patients goals the program is utilizing patient-reported experience measures (PREMs) and patient-reported outcome measures (PROMs) to help assess specific services and show the impact this program can have for persons with stroke. We look forward to supporting this team in reviewing their outcome measures!  

With the addition of Providence Care’s Outpatient Stroke Rehabilitation Program the South East now has three hospital-based Community Stroke Rehabilitation Programs; Providence Care Hospital, Quinte Health, and Perth and Smiths Falls District Hospital.  For persons with stroke who require rehabilitation at home Ontario Health atHome continues to provide Community Stroke Rehab Services throughout the South East to persons with stroke post hospital stay.
