Healthcare Providers from Primary Care and the Stroke Prevention Clinic in the KFLA area came together on June 24, 2024 for a local engagement and education session. The goal of the session was to share information, discuss roles and supports, and identify challenges and opportunities related to primary and secondary stroke prevention.
This event showcased evidence-based practice in stroke care. It provided an overview of current practice across the continuum and offered updates related to primary and secondary prevention, hyperacute and acute care, rehabilitation, and community supports. Collaborative discussions with primary care focused first on challenges and moved into brainstorming opportunities to address these challenges. Some key opportunities that were identified include:
- Empower patients to stay well: “Use every appointment as an opportunity”
- Increase public awareness
- Utilize technology such as prompts/tools/AI within the electronic medical record
- Connect and communicate between programs to share information and build awareness of existing supports.
Survey responses from the 38 individuals who attended indicated many positive take-aways from the event including: a better awareness of outpatient therapy and community support services, the importance of lifestyle in preventing stroke, the value of checking pulse along with blood pressure, and increased awareness and connection to the Stroke Prevention Clinic. Comments from attendees were all positive, stating that the event was “very informative”, and “this was great, can’t wait for the next one!”
